PSYCHOLOGICAL AND EMOTIONAL DISTURBANCES IN PATIENTS WITH DYSCIRCULATORY ENCEPHALOPATHY AND HYPOTHYROIDISM: A PROSPECTIVE STUDY

Introduction. The problem of comorbidity in cardioneurology is one of the urgent problems of the modern health care system. Comorbidity complicates the course of diseases, worsens the prognosis and increases the number of complications. In the case of comorbidity, the main cause of death in the vast majority (94.6%) of patients is cardiovascular and cerebrovascular pathology. In the context of comorbidity, it should be noted that thyroid diseases in Ukraine and other countries have become one of the most common endocrine pathologies that cause a deterioration in the quality of life and work capacity. Hypothyroidism is associated with increased cardiovascular morbidity and mortality. The development of manifestations of dyscirculatory encephalopathy in patients with a neurological profile with signs of thyroid dysfunction is associated not only with a systemic change in blood pressure and peripheral vascular resistance, but also with dyshormonal intracerebral changes. Given the ongoing trend of population aging, the importance of addressing chronic cerebrovascular diseases is growing significantly. Aim. Our study was to investigate psychological and emotional disorders in patients with dyscirculatory encephalopathy patients and hypothyroidism according to the data of a prospective study. Materials and methods. A total of 60 patients with hypertensive dyscirculatory encephalopathy and accompanying hypothyroidism, who were monitored over a 10-year period, were examined. Results. According to the data of a prospective study, in patients with HDE and accompanying hypothyroidism, according to the MMSE scale, deterioration of such indicators as "memory", "attention and calculation", "reproduction of words" is noted. The general indicator of cognitive productivity in the second group (examined patients with HDE and concomitant hypothyroidism at the present time) is statistically significantly worse (p<0.05) than in the first group. Dementia of a mild degree (p<0.05) was detected in patients of the Group II, which reflects the significant negative impact of hypothyroidism on human cognitive functions. According to Beck's depression scale, mild and moderate depression was found in patients of the Group II. Conclusions. According to the results of a prospective study, it was established that concomitant hypothyroidism is a significant factor in the adverse course of hypertensive dyscirculatory encephalopathy in terms of cognitive changes and depressive disorders.
