Technique of Hypofraction Radiation Therapy of Patients with Localized Prostate Cancer. Pre-Radiation Preparation, Delineation, Treatment
Purpose: Improving treatment outcomes of patients with localized prostate cancer (PC). Material and methods:In the radiotherapy department of the N.N. Blokhin NMRCO in the framework of a prospective singlecenter clinical trial of phase I II an original method of hypofractionated RT for patients with localized prostate cancer was proposed and developed, based on radiation exposure to the prostate from 2.7 Gy to 71 Gy (76,9 Gy) with dose escalation to the dominant lesion to 110 iGy with simultaneous prophylactic radiotherapeutic exposure to the pelvic lymph nodes from 2.2 Gy to 44 Gy (45.6 Gy). Results: From XI/2023 to V/2024, 24 patients with localized prostate cancer at unfavorable (n=5), high and very high risk of progression (n=19) with a median followup 6 (312) months were treated. All patients received treatment according to the developed original technique. At the time of completion of RT there were symptoms of grade I II toxicity from the genitourinary system (GU), the distal gastrointestinal tract (GI), which were recovered 6 months after the end of RT. No toxicities of grade III IV were noted in one patient (according to the RTOG/EORTC and NCI CTCAE scales. Conclusion: Initial data of the present trial, assessing the safety of the original technique of hypofractionated RT, demonstrate good tolerability. However continuation of the ongoing prospective clinical trial is required to determine the effectiveness of the treatment.
