Giant Abdominal Hernias in Obese Patients: Challenges and Treatment Strategies
Introduction Giant abdominal hernias in obese patients present a unique and growing surgical challenge, characterized by anatomical distortions, functional impairments, and increased perioperative risk. Objective To evaluate the challenges and treatment strategies for giant abdominal hernias in obese patients, with an emphasis on surgical techniques, perioperative optimization, and interdisciplinary care. Methods A narrative review was conducted through an extensive search of PubMed, Scopus, Web of Science, Cochrane Library, and Google Scholar, focusing on literature published between 2018 and 2023. Inclusion criteria targeted adult obese patients undergoing surgical hernia repair. Studies were thematically categorized and critically analyzed. Results and Discussion Preoperative conditioning, component separation techniques, mesh selection, and multidisciplinary approaches emerged as key elements in successful hernia repair. Despite technical advancements, complications remain frequent, and long-term durability varies. Psychosocial support and structured postoperative care are essential to optimize outcomes. Conclusion Effective management of giant hernias in obese patients requires a holistic, multidisciplinary, and evidence-based approach. Future research should address knowledge gaps in surgical technique efficacy, long-term outcomes, and patient-centered care strategies.
