Bệnh liên quan đến IgG4: Hai trường hợp lâm sàng và tổng quan y văn
Immunoglobulin G4-related disease (IgG4-RD) is an immune-mediated fibroinflammatory condition that can affect multiple organs. Here we present two cases of IgG4-RD that were diagnosed using imaging, serum IgG4, and histology. The first case was a 58-year-old man with a history of gallbladder mass resection and pancreatic pseudocyst drainage surgery who presented with biliary obstructive symptoms and recurrent pancreatitis. CT/MRI revealed diffuse sausage-like swelling of the pancreas. His histopathology and serum IgG4 were typical of IgG4-RD, and the patient responded rapidly to corticosteroids. The second case involved a 77-year-old man with a history of idiopathic pancreatitis who presented with biliary obstructive symptoms and acute recurrent pancreatitis. An abdominal CT scan showed diffuse sausage-like swelling of the pancreas, sclerosing cholangitis with thickening and enhancement of the common bile duct wall. His serum IgG4 level was highly elevated, and the patient was diagnosed with IgG4-RD. He responded well to corticosteroid therapy. IgG4-RD is generally seen in males in the sixth to seventh decade of life and can involve one or multiple organs. It should be considered in patients presenting with suspect symptoms.
