Navigating Infertility in Unicornuate Uterus: Clinical Management and Success with Assisted Reproductive Techniques
A BSTRACT A 36-year-old female with primary infertility and a history of a unicornuate uterus sought treatment at a Test Tube Baby Centre in Wardha, Maharashtra, India. Despite four failed intrauterine insemination (IUI) attempts, diagnostic laparoscopy and magnetic resonance imaging (MRI) confirmed her congenital uterine anomaly. Hormonal analysis revealed an anti-Müllerian hormone (AMH) level of 0.79 ng/mL and a follicle-stimulating hormone (FSH) level of 18 IU/mL, indicating diminished ovarian reserve. Controlled ovarian stimulation using gonadotropin-releasing hormone (GnRH) agonists and antagonists resulted in the retrieval of eight oocytes, of which three were mature (MII). Intracytoplasmic sperm injection (ICSI) was performed, leading to the development of a top-quality blastocyst (4AA) that was transferred into the uterus. Fourteen days post-transfer, serum beta-human chorionic gonadotropin (β-hCG) levels confirmed pregnancy (1300 mIU/mL). This case highlights the significance of individualized In Vitro Fertilization (IVF) protocols in overcoming infertility associated with uterine anomalies, ultimately leading to a successful conception.
