Local episcleral filling in patients with regmatogenic retinal detachment

Despite the constant improvement of intraocular methods of treating vitreoretinal pathology, in surgery for regmatogenic retinal detachment, the most gentle, extrascleral methods remain quite relevant. The aim of the work was to study the results of extrascleral local filling in patients with regmatogenic retinal detachment. Material and methods. Patients who underwent surgery for regmatogenic retinal detachment were retrospectively observed. There are 36 patients in total. In all cases, patients were hospitalized up to 12 days after the disease with a diagnosis of regmatogenic retinal detachment, PVR, stage A. Corrected visual acuity before surgery ranged from 0.04 to 1.0. It was mandatory to examine with a 3-mirror lens for more accurate localization of ruptures. The technique of the operation. After local anesthesia and access to the area of the suspected indentation, the rupture was searched for and projected onto the sclera using a sclerocompressor and a 78-diopter noncontact lens using an operating microscope. The silicone seal was fixed with U-shaped seams. At the same time, the maximum length of the indentation zone was 5 hours. Results. Blocking of the rupture and complete fit of the retina was achieved in 31 patients, which was 86 %. Rupture blocking was observed in the period from 1 to 5 days. After that, transpupillary retinal laser coagulation was performed. The duration of complete retinal fit ranged from 4 days to 6 months. The corrected visual acuity after surgery remained at the same level in patients with 100 % vision and improved in all other patients and ranged from 0.2 to 0.6. Conclusions. 1. Extraocular surgery remains the least traumatic and prognostically more favorable method of treatment for «fresh» retinal detachment. 2. Thorough preoperative preparation (rest test, repeated examinations for more accurate localization of ruptures) allows to maximize the anatomical and functional result. Keywords: retinal detachment, episcleral filling, extraocular surgery
