A STUDY OF PERCUTANEOUS NEPHROLITHOTOMY (PCNL) AND GRADING OF COMPLEXITY OF PCNL PROCEDURES USING “GUY'S STONE SCORE

Background: Before performing a percutaneous nephrolithotomy, various scoring systems were established for the purpose of preoperatively predicting stone-free status (SFS) and complications. These systems were constructed by evaluating the intricacy of renal stones. The four nephrolithometry scoring systems that are utilised most often in the modern era are the Guy's stone score (GSS), the S.T.O.N.E. nephrolithometry score, the nomogram developed by the Clinical Research Ofce of the Endourology Society (CROES), and the Seoul National University Renal Stone Complexity (S ReSC) score. For the purpose of being able to be utilised in day-to-day practice, Guy's stone score was designed to be rapid, straightforward, and repeatable. Hence, this study aimed to assess the utility of GUY's score following PCNL. Methods And Material: 50 patients with symptomatic renal stones presenting to Urology OPD are evaluated and included in the study after informed consent. The indications for surgery are studied and patient is taken up for the same after anaesthetic tness. PCNL is done using standard techniques. The complexity of procedure is graded using radiological studies and the outcome assessed based on “Guy's Stone Score” and modied Clavien system. Guy score was signicantl Results: y associated with the increased operative time, more bleeding and a higher Clavein Dindo grade. Guy score also was found to be higher in those with retained stones, requiring additional surgical procedures. This study was p Conclusion: erformed to assess the utility of GUY score to evaluate the outcomes of PCNL. Guy score was a good predictor of unfavourable outcomes in our study such as prolonged surgical duration and a poorer post-operative performance. By evaluating the Guy score, we can also predict the possibility of retained stones, and need for ancillary procedures.
