Introduction: Aseptic osteonecrosis of the talus is a bone ischemia of the talus responsible for a reduction or total loss of blood supply to the talar bone. It is rare and represents 2% of all aseptic osteonecroses. The aim of this work was to study the radiological contribution in talar necrosis through two observations. Observations : It was a 21-year-old woman with no known medical-surgical ATCDS and a 22-year-old man with a history of osteonecrosis of the femoral head, all referred to the medical clinic "Marie Curie" in Bamako for the frontal and lateral radiography of the right ankle in both patients and the CT scan in the patient. Clinically, the patient had gait disturbance and painful swelling with deformation of both ankles, more marked on the right, and the patient had pain with functional impotence of the left foot. X-ray and CT scan had found the appearance of talar necrosis in both observations. We had classified according to Ficat in stage IV at the level of the right ankle and stage III at the level of the left ankle of the patient and stage II of the right ankle of the patient. Conclusion : Aseptic osteonecrosis of the talus is a rare pathology of the talus and can be due to traumatic and non-traumatic injuries. X-ray and CT scan are essential examinations in therapeutic management in our country.
