Expanding the Role of Novel Therapeutics in Cardiometabolic Syndrome: Beyond Heart Failure and Diabetes
Cardiometabolic syndrome (CMS) is a global health concern, characterized by the interplay of cardiovascular, metabolic, and renal disorders, such as hypertension, dyslipidemia, hyperglycemia, and central obesity.Traditional management approaches often fail to address the complexity of CMS, highlighting the need for innovative therapies with broader efficacy.Angiotensin receptor-neprilysin inhibitors (ARNIs) and sodium-glucose cotransporter-2 (SGLT2) inhibitors have emerged as transformative agents, providing substantial cardiovascular, metabolic, and renal benefits.ARNIs modulate the renin-angiotensin-aldosterone system and enhance natriuretic peptides, whereas SGLT2 inhibitors improve glycemic control, promote weight loss, and offer hemodynamic advantages.Combination therapy with ARNIs and SGLT2 inhibitors produces synergistic effects, further amplifying clinical benefits, although challenges such as hypotension, hyperkalemia, and cost remain.Emerging therapies, including glucagon-like peptide-1 receptor agonists and novel mineralocorticoid receptor antagonists, show promise in expanding treatment options.Future research should emphasize personalized medicine approaches, overcoming implementation barriers, and investigating the long-term impacts of combination therapies. This review highlights the evolving therapeutic landscape of CMS, offering insights into optimizing management strategies and improving patient outcomes in this multifaceted syndrome.
