Safety and efficacy of myomectomy: results of a single-center cohort study

Aim: to evaluate the outcomes of myomectomy and the incidence of intraoperative and postoperative complications Patients and Methods: between January 2019 and December 2022, a total of 268 myomectomies were performed. The surgeries utilized both laparoscopic and laparotomic techniques A database was developed to collect and analyze information on the female patients included in the study. The data were collected retrospectively by reviewing medical records and patient questionnaires. Among the 268 patients, complete data were obtained for 219. Results: the size of the fibroids ranged from 4 to 25 cm, with an average diameter of 9.02 cm. Uterine cavity deformities were observed in 56/268 (20.89%) patients, and uterine cavity dissection was noted in 52/268 (19.4%). A correlation was identified between the volume of blood loss and both the number of removed fibroid nodes (p=0.072) and the diameter of the largest node (p<0.01). In the postoperative period: 10/268 (3.73%) patients experienced dynamic bowel obstruction, which resolved spontaneously; 18/268 (6.72%) patients had a fever; 13/268 (4.85%) patients developed lower urinary tract infections; 1/268 (0.37%) patient experienced suture site suppuration; 1/268 (0.37%) patient developed a postoperative hernia. During follow-up, 32/219 (14.61%) female patients experienced recurrence of fibroids, of whom 1/219 (0.46%) underwent repeated surgery. No intraoperative complications, such as bladder or bowel injuries, or unplanned hysterectomies were reported. At the time of the survey, 35/219 (15.98%) women successfully restored their reproductive function, achieving pregnancy and delivery. All children born were healthy. No cases of uterine scar failure were identified intraoperatively or via follow-up pelvic ultrasound. Conclusion: laparoscopic myomectomy is a safe and effective surgical treatment for uterine fibroids, with a low incidence of complications, even during subsequent pregnancies KEYWORDS: uterine fibroids, laparoscopic myomectomy, pregnancy, complications, laparoscopy, intraoperative complications, postoperative complications FOR CITATION: Slobodyanyuk B.A., Dobrokhotova Yu.E., Dimitrova V.I., Markova E.A., Slyusareva O.
