Detangling Ichthyosis

Ichthyosis represents a broad spectrum of skin disorders with varying characteristics and severity. While we may be able to diagnose ichthyosis as a general condition, pinpointing an exact type or conclusion can often be challenging due to its complex nature. Diagnosing ichthyosis is challenging due to its complexity, as it involves the skin, its appendages, and internal organs, making the diagnostic process intricate. Distinguishing between different clinical types of ichthyosis can be confusing, particularly for beginners and busy practitioners, as the disease presents in both congenital and acquired forms. Some cases are associated with the involvement of other organs, leading to syndromic presentations that further complicate diagnosis. In addition, ichthyosis may present with a range of skin features, such as photosensitivity, hyperlinear palms, bullae, and erythroderma, and can also affect hair and other appendages. The involvement of internal organs, such as the heart, lungs, central nervous system, and kidneys, adds to the complexity, as practitioners must assess both cutaneous and systemic abnormalities. Despite the aids provided by textbooks, such as tables and flowcharts, diagnosing ichthyosis remains difficult due to the wide range of clinical presentations and the need for thorough, multisystem evaluation. Recognizing this difficulty, an effort has been made to simplify the theoretical understanding of ichthyosis. This approach aims to offer a clearer pathway toward a more accurate diagnosis, enhancing both the comprehension and management of this condition [Flowcharts 1-3 and Tables 1-3].[1,2]Flowchart 1: Classification of ichthyosisFlowchart 2: (a) Classification of ichthyosis as per age of onset and cutaneous findingsFlowchart 3: Classification of ichthyosis as per different organs involvedTable 1: Different hair disorders associated with ichthyosisTable 2: Different ocular disorders associated with ichthyosisTable 3: Acronyms for syndromesDeclaration of patient consent The authors certify that they have obtained all appropriate consent forms, duly signed by the parent(s)/guardian(s) of the patient. Abstract
