JUSTIFICATION OF THE CONCEPTUAL, FUNCTIONAL, AND ORGANIZATIONAL MODEL OF THE REGIONAL CARDIAC CARE SYSTEM FOR THE ADULT POPULATION
Connection of the publication with planned research work. This study is a fragment of the research project "Medical and social substantiation of optimization of medical care in the context of health care system development", state registration number 012U100807.Introduction.Cardiovascular disease (CVD) remains the leading cause of mortality and disability worldwide, necessitating improvements in the cardiac care system.According to the WHO Global Plan of Action for the Prevention and Control of Noncommunicable Diseases 2013-2030 [1] and the WHO European Strategy for Cardiovascular Health [2], countries should develop integrated, patient-centred models of cardiac care that prioritize accessibility, efficiency, and intersectoral collaboration. In Ukraine, the strategic framework for healthcare system development is defined by the National Healthcare Strategy of Ukraine for the Period up to 2030 [3], which envisions the implementation of modern approaches to medical care based on European standards. The regulatory and legal framework for cardiac care is established, in particular, by the Order of the Ministry of Health of Ukraine (MoHU) No. 975 of 29.08.2017,On Improving the System of Cardiac Care in Healthcare Institutions of Ukraine [4].At the same time, healthcare financing reform, decentralization of management, and the establishment of hospital districts necessitate the adaptation of organizational models to ensure the provision of specialized cardiac care in response to current challenges. An analysis of scientific studies indicates that optimizing regional cardiac care systems contributes to reducing cardiovascular mortality and improving patients' quality of life.Research conducted by the European Society of Cardiology (ESC) [5] and the American Heart Association (AHA) [6] confirms the effectiveness of integrated models that incorporate primary prevention, early diagnosis, advanced treatment methods, and rehabilitation.Given the above, it is essential to provide a scientifically substantiated conceptual, functional, and organizational model for the regional cardiac care system for the adult population. This model aligns with international standards, current regulations, and research findings, ensuring the accessibility and effectiveness of cardiac care at all levels of the system. The aim of the study. To enhance the regional medical care system for the adult population in the fields of cardiology and cardiac surgery by substantiating its conceptual, functional, and organizational model.Object and research methods. The study focused on the system of cardiology and cardiac surgery care for the adult population of the Kyiv region at the territorial and local levels during the period Cardiovascular diseases (CVD) remain the leading cause of mortality worldwide, necessitating improvements in cardiac care. The World Health Organization (WHO) recommends comprehensive approaches to the prevention, diagnosis, treatment, and rehabilitation of patients with CVD.In Ukraine, the Healthcare Development Strategy until 2030 envisions decentralization and the establishment of hospital districts.Developing a scientifically grounded model of a regional cardiac care system that aligns with international standards and strategies for the innovative development of Ukrainian regions will enhance the accessibility and quality of cardiac care for the adult population. The aim of this study was to improve the regional system of medical care for the adult population in the fields of cardiology and cardiac surgery by substantiating its conceptual, functional, and organizational model. The study focused on the system of cardiology and cardiac surgery care for the adult population of the Kyiv region at the territorial and local levels from 2018 to 2022.The following methods were employed: bibliosemantic analysis, systemic and structural-logical analysis, and conceptual modeling. The cardiac care system in the Kyiv region faces several challenges, including a high prevalence of CVD, a shortage of medical personnel, and outdated infrastructure. To address these issues, a model has been proposed based on strategic and cluster approaches, as well as the integration of cardiology and cardiac surgery care. This model involves rational resource management, modernization of equipment, optimization of patient pathways, and the creation of educational platforms. The system will be coordinated by a regional cardiology center, ensuring the equitable distribution of resources, continuous provision of medical services, and compliance with international standards. The proposed model of regional cardiac care enhances its effectiveness by integrating healthcare institutions, establishing a cardiac cluster, and fostering intersectoral cooperation.Collaboration with research and educational institutions facilitates the implementation of innovations in medical practice. An integrated approach ensures accessibility and high-quality medical services, contributing to improved public health and a reduced burden of CVD.
