An Unusual Case of COVID-19 Enterocolitis Complicated by Promyelocyte Arrest and Pancytopenia

Abstract An adolescent boy presented with acute bloody diarrhea which was not resolving on empirical antibiotics. On detailed evaluation and workup to identify the culprit organisms, the rectal swab polymerase chain reaction turned to be positive for coronavirus disease 2019 (COVID-19). This was further complicated by the development of pancytopenia. Bone marrow study revealed promyelocyte arrest and reduced maturation of all cell lineages. With the suspicion of COVID-19-associated enterocolitis and pancytopenia, the patient was started on steroids. The patient improved on the addition of steroids without any antibiotic coverage. This is an unusual presentation of COVID-19. The patient has recovered from his illness and is back to his normal life.
