60 Reviewing the impact of gold standards framework on patient place of death

Introduction
Gold Standards Framework (GSF) supports clinicians in identifying patients in their last year of life to aid in the development of proactive and personalised plans for care that includes discussions about preferred place of care and death. At Dudley Group NHS Foundation Trust (DGFT) the GSF has been implemented trust-wide and this allows an opportunity to evaluate the impact that implementation has had on a patients place of death.
Methods
All adult inpatients on 01/09/2023 at DGFT who died in the next 12 months in any setting were retrospectively reviewed to determine if on 01/09/23 the patients had a recorded GSF Code, their date of death (if applicable) and place of death (hospital or not hospital). Patients identified as GSF Red were excluded, owing to the short prognosis for these patients following their identification.
Results
Out of 647 inpatients on 01/09/2023, 216 patients died in the following 12 months. 112 patients who died were not GSF identified on 01/09/2023 whilst 104 had a recorded GSF Code. The 33 patients identified GSF Red were excluded, leaving 71 patients identified as GSF Amber or Green. For patients who were identified GSF Amber or Green on 01/09/2023, 35% died in hospital and 65% died outside of hospital. In comparison, of the 112 patients who died and were not GSF identified on 01/09/2023; 55% of these patients died in hospital and 45% died outside of hospital.
Conclusion
The review has shown that patients who are identified and coded GSF Amber or Green were less likely to die in a hospital setting in the year following identification compared to those who were not identified (35% vs 55%). This highlights the beneficial role GSF implementation has in supporting patients preferences around place of death.
