142 An audit studying palliative care provisions in the community

Objectives studied in this audit focused on the use of the Gold Standard Framework (GSF) in a general practice (GP) setting and the consequences of usage on palliative care practices, measured using various factors including advance care planning use and adherence to the patient’s desired place of death. The sample used was selected based on their place on a palliative care register at the audited General Practice (GP) and their date of death being between February – November 2023. Data collected to measure these factors comprised the patient’s place of death (desired and actual), the palliative care services used and their reason for palliation, which was then compared to data on GSF use for each patient to investigate potential relationships. Some key findings included the increased number of patient deaths in their preferred place and the overall reduction in the number of hospital deaths with use of the GSF. A further finding was the overall lower-than-expected uptake of palliative care services, GSF and advance care plans. In conclusion, community use of the GSF by healthcare providers has shown to have positive impacts on community palliative care outcomes across multiple measured factors.
