114 NI hospice pressure ulcer prevention group

Background
The Pressure Ulcer Prevention group was set up in NI Hospice as a multidisciplinary group (nursing, healthcare assistants and OT) to consider all aspects of pressure relief in promoting high quality patient care. Hospice patients are a vulnerable group, with multiple risk factors which can make them more susceptible to pressure damage, particularly towards end of life.
Method
Key aspects of the group: Documentation: SSKIN bundle introduced in 2019 Education: Pressure ulcer prevention booklet for patients and relatives; Staff information leaflet; Mattress flow chart to assist staff in choosing appropriate mattress for each patient; Educational display stand twice a year, one of which coincides with International Stop The Pressure Day Equipment: Regular review of equipment in use and new equipment available including trials; All patient seating allows for integrated pressure relief; Pressure reducing cushions for patient garden seating; Lateral turning system (Toto); Smaller aids available are mattress infills, ear guards for oxygen tubing and KerraPro strips Audit: Monthly SSKIN bundle audits completed and regular audits on mattresses, sliding sheets and cushions
Results
The introduction of the SSKIN bundle, implementation of equipment and staff education has resulted in a significant reduction in Hospice acquired pressure ulcers. Figures from April 2022 to March 2024 showed a 42.6% reduction in pressure ulcers (per occupied bed day).
Conclusion
The holistic approach in aiming to reduce pressure ulcers has had a significant impact on the ward and is very much a work in progress. Regular auditing allows the group to check areas that need addressed, equipment needing reviewed/purchased or new aspects that we need to consider, develop or provide education. We strive to further improve in all areas, providing the highest quality care for our patients.
