The State of Immunity and Methods of Its Correction in the Preoperative Period in Patients with Cystocele against the Background of Chlamydial Infection of the Pelvic Organs and Other Urogenital Organs

Purpose. To propose an adequate method of immunocorrection in the preoperative period in patients with cystocele on the background of chlamydial infection of the pelvic organs and other urogenital organs. Materials and methods. The study was based on the examination and treatment of 40 patients (main group) aged 51.1±3.2 years who came to us for the elimination of cystocele, in whom chronic chlamydiosis (CH) was detected. They were divided into two groups (A and B) of 20 patients each. The control group C consisted of 20 patients with cystocele without any chronic infection. In groups A and B, immunocorrecting therapy (in group A – immunofan, in group B – immunofan + 0.33% saffron extract) and antibiotic therapy were carried out for 14 days. Indicators of cellular and humoral immunity were studied. Results. The immune status of patients with CH was characterized by the presence of suppression in the T-cell link and imbalance of humoral immunity indicators. After immunocorrection, the effectiveness of immunocorrection with immunofan + saffron was 20% higher than in the group where immunocorrection was carried out only with immunofan. In group B, good results of cystocele correction were 4.7% lower than in the control group; in group A, this indicator was at the level of 6.23%. Conclusion. The presence of chronic chlamydiosis in patients with cystocele is an indication for the appointment of eradication antibacterial and immunocorrecting therapy in the preoperative period. Carrying out immunocorrection with immunofan in combination with 0.33% saffron extract is more effective. Цель. Предложить адекватный метод иммунокоррекции в предоперационном периоде у пациентов с цистоцеле на фоне хламидийной инфекции органов малого таза и других урогенитальных органов. Материалы и методы. Обследование и хирургическое лечение прошли 40 пациентов (основная группа) в возрасте 51,1±3,2 года с цистоцеле, протекающим на фоне хронического хламидиоза. Они были разделены на 2 подгруппы (А и Б) – по 20 человек в каждой. В контрольную группу В вошли 20 пациентов с цистоцеле, не имеющих какой-либо хронической инфекции. В группах А и Б иммунокоррекцию (в группе А проводили с использованием иммунофана, а в группе Б – иммунофана в сочетании с 0,33%-й настойкой шафрана) и антибиотикотерапию проводили на протяжении 14 дней. Исследованы показатели клеточного и гуморального звеньев иммунной системы. Результаты. Состояние иммунитета пациентов с хламидийной инфекцией характеризовалось наличием супрессии в Т-клеточном звене и дисбалансом показателей гуморального иммунитета. После иммунокоррекции иммунофаном + шафраном ее эффективность была на 20% выше, чем в группе, где иммунокоррекция проводилась только иммунофаном. В группе Б результаты коррекции цистоцеле были на 4,7% ниже, чем в контрольной группе, в группе А этот показатель был на уровне 6,23%. Заключение. Наличие хронического хламидиоза у пациентов с цистоцеле является показанием для назначения эрадикационной антибактериальной и иммунокорригирующей терапии в предоперационном периоде. Проведение иммунокоррекции иммунофаном в сочетании с 0,33%-м экстрактом шафрана является более эффективным.
