EFFECTIVENESS OF THE COMPLEX TREATMENT OF PATIENTS WITH PULMONARY TB AND CONCOMITANT TRACHEOBRONCHIAL TB

Objective: to evaluate effectiveness of the complex treatment of patients with pulmonary tuberculosis (PTB) and concomitant tracheobronchial TB (TBTB) using pathogenetic therapy. Materials and methods. The study enrolled 60 patients divided into two groups. The main group included 30 patients with PTB and TBTB, who received the complex treatment containing both chemotherapy and pathogenetic therapy (corticosteroids, inhalations and lymphotropic therapy). The comparison group included 30 patients with similar diseases (a retrospective analysis), who only received chemotherapy. Results. The patients of the main group demonstrated more rapid cessation of bacterial excretion by month 2 of treatment based on sputum microscopy results – in 72,2% of patients against 41% in the chemotherapy group (p < 0,05). A significantly higher incidence of tracheobronchial tree TB cure was also observed in the main group vs the comparison group: 50% vs 10% by the 2nd month of treatment (р = 0.002, OR = 9 (95% CI 2.24–36.17)), and the tendency to less frequent development of scarring airway stenosis. Conclusion. The complex treatment of PTB and TBTB using pathogenetic methods contributes to more rapid cessation of bacterial excretion according to both sputum microscopy and culture methods in the early stages of therapy, and also to more rapid cure of TBTB.
