Relation of hypertensive retinopathy and coronary artery disease

ABSTRACT Hypertension (HTN) is the primary risk factor for both cardiovascular disease (CVD) and death. This disease has a major effect on the vasculature of the eye’s anatomy and function, leading to various pathophysiological changes in the retina, choroid, and optic nerve circulation due to increased blood pressure. These alterations lead to several clinical presentations, the most prevalent form being the hypertensive retinopathy (HR), resulting from blood pressure spikes that are both acute and long term. Both HR and coronary artery disease (CAD) are the vascular complications connected to persistently high blood pressure. Since retinal blood vessels mirror the state of similarly sized vessels in many other organs, one of them being the heart, they provide significant prognostic insights into vascular health. The aim of this article is to understand the relation between HR and CAD. A literature review was conducted using “Google Scholar,” “Medline,” and “PubMed.” Pathophysiological changes in the retina associated with HTN have been suggested as a means to identify individuals at risk for CVD. Most global HTN management guidelines highlight that the presence of HR, particularly in conjunction with renal failure and left ventricular hypertrophy, signifies target organ damage and necessitates more intensive treatment for these patients. Some research indicates that HR is a strong and separate risk factor for the development of CAD in hypertensive individuals. In addition, signs of HR can predict clinical events related to CAD and congestive heart failure. Thus, there exists a substantial correlation between the severity of CAD as observed in coronary angiography and the greater grade of HR.
