The relationship between psycho-emotional status and pain intensity after orthognathic surgery and their impact on early postoperative rehabilitation

Objective. To analyze the dependence of pain intensity and early rehabilitation, as well as length of hospital-stay on extent of surgery and psychological state of patients; to estimate the influence of postoperative pain syndrome on comfort of patients in early rehabilitation period and duration of hospital-stay. Material and methods. We analyzed VAS scores of pain and comfort, as well as length of hospital-stay depending on orthognathic surgery and severity of psycho-sensory-anatomical-functional (PSAF) auto-maladjustment syndrome. Pain intensity and comfort were assessed on the 2nd postoperative day. Results. When comparing VAS scores of pain and comfort, as well as length of hospital-stay depending on orthognathic surgery, we found no significant between-group differences (p<0.05). Distribution of patients into subgroups depending on PSAF auto-maladjustment syndrome revealed significant differences (p>0.05) when comparing VAS scores of pain in all groups, VAS scores of comfort in all groups except for S Le Fort I / BSSO / Genioplasty and BSSO groups, as well as length of hospital-stay in all groups except for BSSO / Genioplasty (p>0.05). There was significant positive strong relationship between VAS scores of pain and comfort, as well as VAS scores of pain and length of hospital-stay. Conclusion. Psycho-emotional status and pain intensity influence on early postoperative rehabilitation after orthognathic surgery. At the same time, extent of surgery has no significant effect on pain intensity, comfort in early rehabilitation period and duration of hospital-stay.
