專科護理師在「在宅急症照護(Hospital at Home)」的角色

<p>專科護理師在「在宅急症照護」（Hospital at Home, HaH）中發揮了關鍵作用，從篩檢適合HaH的病患、擔任轉銜護理師，到進行居家護理訪視及處理緊急情況。通過與醫師和其他醫療團隊的合作，專科護理師在病人家中提供與醫院等級相當的照護服務，有效減少急診壅塞，提升醫療資源的利用效率。這不僅改善了急性病人的居家治療質量，還幫助減輕家屬照護壓力，為高齡化社會提供了一種具可行性的人性化醫療選擇。</p> <p> </p><p>Nurse practitioners (NPs) play a pivotal role in the “Hospital at Home” (HaH) model. Their responsi-bilities include screening patients for HaH eligibility, serving as transitional care nurses, conducting home care visits, to managing emergencies, NPs deliver hospital-level care in patients’ homes, effec-tively reducing emergency department overcrowding and optimizing healthcare resource utilization. This model not only enhances the quality of home-based acute care but also alleviates the caregiving burden on families, providing a practical and compassionate healthcare solution for an aging society.</p> <p> </p>
