Optimizing LDL-C Reduction: High-Dose Pitavastatin Vs. Combination Therapy With Ezetimibe In Type II Diabetes
Background: Patients with type II diabetes are at an increased risk of cardiovascular disease, making LDL-C reduction a key therapeutic goal. While high-dose statins are commonly used, combination therapy with ezetimibe may offer enhanced efficacy with fewer adverse effects. Objective: To compare the efficacy and safety of high-dose pitavastatin versus pitavastatin plus ezetimibe in optimizing LDL-C reduction in patients with type II diabetes. Study Design & Setting: This was a randomized, controlled study conducted at a tertiary care hospital over 12 weeks. Methodology: A total of 145 patients with type II diabetes and dyslipidemia were randomly assigned to receive either high-dose pitavastatin (n=72) or a combination of pitavastatin with ezetimibe (n=73). Lipid profiles, including LDL-C, HDL-C, triglycerides, and total cholesterol, were measured at baseline and after 12 weeks. Adverse events were also recorded. Results: The combination therapy group achieved significantly greater LDL-C reduction (-46.1 ± 8.3%) compared to the high-dose pitavastatin group (-39.4 ± 7.8%) (p < 0.001). HDL-C improvement (p = 0.003) and triglyceride reduction (p = 0.02) were also more pronounced in the combination group. Adverse events, including myalgia and elevated liver enzymes, were comparable between groups (p > 0.05). Conclusion: Pitavastatin plus ezetimibe provided superior LDL-C reduction and lipid profile improvements without increasing adverse events, making it a viable alternative to high-dose statin therapy in type II diabetes.
