A system for supporting expert decision-making on the choice of technical means of rehabilitation for patients with breast cancer
Objective. Malignant neoplasms remain the leading cause of disability in Russia, and breast cancer occupies a leading place among oncological pathology in women. The medical and social examination of such patients includes the development of an individual rehabilitation program (IRP), which includes the selection and appointment of technical means of rehabilitation (TMR). Creating a system to support expert decision-making for the choice of TMR for these patients is an urgent task in the field of medical and social expertise. To develop an expert decision support system to assist expert doctors in recommending TMR to patients with breast cancer. Material and methods. The study used a dataset containing depersonalized information from the ITU and IRP acts of 1,572 patients with breast cancer who underwent a medical and social examination in 2023–2024 in St. Petersburg. The analysis of TMR included in IRP in patients with breast cancer was performed, the relationship between the appointment of TMR and the clinical parameters of these patients was studied. Based on the analysis and indications for the appointment of TMR, a digital artificial intelligence model has been developed for recommendations on the inclusion of TMR in IRP programs in these patients. This model has been integrated into the expert decision support system. Results. The most commonly prescribed products for patients with breast cancer are breast exoprostheses and knitted covers for them, a bra or grace to fix them, an orthopedic bandage on the upper limb to improve lymphovenous outflow. According to the calculated metrics, the developed artificial intelligence model based on decisive rules showed high quality of work when making recommendations (the proportion of correct predictions is 91–95 %, the f-measure is 92–96 %). Based on this model, a system has been developed to support expert decision-making on the selection of TMR for patients with breast cancer. Conclusion. The developed system for supporting expert decision-making on the choice of TMR for patients with breast cancer can be effectively used in the activities of expert doctors.
