Portrait of an ischemic heart disease patient with low rehabilitation potential after coronary artery bypass grafting upon admission to the second stage of rehabilitation

Objective. Comparative assessment of clinical, anamnestic and anthropometric data, echocardiography, coronary angiography and laboratory tests in patients with coronary artery disease who underwent coronary artery bypass grafting, depending on their rehabilitation potential. Material and methods. Screening of discharge summaries of medical records of 118 patients with coronary artery disease aged 36 to 85 years (mean age 63.3 + 2.0 years) was conducted. Among the examined patients, there were 86 men and 32 women. We assessed the RP using the method we developed. Of the 118 examined patients, 27 had high RP, 50 had medium RP, and 41 had low RP. To achieve the goal of the study, we compared clinical, anamnestic and anthropometric data, echocardiography, coronary angiography and laboratory tests in patients with low RP and patients with high RP. Results. Patients with coronary heart disease with low RP who underwent coronary artery bypass grafting are characterized by a severe, progressive course of the disease, manifested by dyslipidemia, the presence of asynergy zones, impaired contractility of the left ventricular myocardium, a significant mass of the left ventricular myocardium and a low relative thickness of its wall, which indicates eccentric myocardial hypertrophy as a predominant variant of cardiac remodeling. Conclusion. With the help of screening for rehabilitation potential, which is determined by mathematical modeling, it became possible to predict the condition of patients with coronary heart disease after coronary artery bypass grafting at the 1st stage of rehabilitation. Detection of low RP should be the basis for an individual approach to the organization of rehabilitation treatment. Physical activity, without which full rehabilitation is impossible, may be inadequate in this contingent, which can lead to complications.
