The use of laser therapy to improve microcirculatory processes in mechanical jaundice

Objective. The aim of the work is to study the effects of laser therapy on microcirculatory processes in patients with obstructive jaundice. Material and methods: A clinical and laboratory study of 40 patients with obstructive jaundice was conducted. The patients were divided into groups according to the classification of V. D. Fedorov et al. (2000): mild severity (persons with bilirubin levels up to 100 mmol/l and other criteria); average (persons with bilirubin levels from 100 to 200 mmol/l and other criteria). 4 groups of patients were formed: Group I (n = 10) — patients with mild jaundice with standard treatment; group II (n = 10) — patients with moderate jaundice with standard treatment; Group I (n = 10) and group II (n = 10) — patients with mild to moderate severity on the background of standard treatment and laser therapy. Laser therapy included daily sessions of procedures for 9 days in the projection of the cubital vein for 15 minutes, as well as in the projection of the common carotid and vertebral arteries for 5 minutes on each side (Matrix device, CLOZ head (radiation with a wavelength of 635 nm,power of 2 MW). To verify microcirculatory changes, laser Doppler flowmetry was used at bioactive point E-36 on days 1, 3, 5, and 9. Statistical processing was performed using the IBM SPSS Statistics 22 program. Results. When analyzing these microcirculation parameters (M, Max CF1/Amax LF), recovery of microcirculatory disorders on the 5th and 9th days with mild and moderate severity was noted in patients with complex therapy. When evaluating these indicators of HT and PS, recovery of microcirculatory disorders was noted with the inclusion of laser therapy on the 9th day in patients with mild severity. Conclusion. The use of laser therapy improves microcirculatory processes in patients with obstructive jaundice, which allows its use in clinical practice. It is worth emphasizing that the effectiveness of laser therapy and the severity of microcirculation disorders depend on the level of liver damage and the severity of jaundice. The inclusion of supravascular laser blood irradiation in the complex treatment led to the restoration of microcirculation on the 5th and 9th days, respectively. Thus, laser therapy in the complex treatment of patients with obstructive jaundice contributes to the patient’s recovery in the shortest possible time.
