Interplay of neuroendocrine tumor progression, chronic alcohol use, and depression: A complex case analysis and multidisciplinary approach

Neuroendocrine neoplasms are a diverse group of tumors with hormone-secreting potential, affecting patient quality of life and prognosis. This report discusses a 56-year-old male with a likely intestinal-origin neuroendocrine tumor and a 20-year history of heavy alcohol use, who developed a major depressive episode following his cancer diagnosis. Diagnostic imaging and histology confirmed a well-differentiated neuroendocrine tumor (NET G1), and treatment with octreotide and everolimus was initiated due to disease progression. Despite treatment, he developed gastrointestinal complications and persistent endocrine disturbances, requiring recurrent ascitic drainage. This patient’s depressive episode appears multifactorial, with contributions from neuroendocrine and alcohol-related neurotoxic effects, compounded by psychological distress over his cancer prognosis. A multidisciplinary approach is crucial for managing both his oncologic and psychiatric symptoms. This case highlights the need for further research into the complex interaction between neuroendocrine neoplasms, mood disorders, and substance use, aiming to improve management and quality of life for similar patients.
