Correction of zygomatic area in orthognathic procedures

Objective. To evaluate zygomatic changes following reduction and augmentation cheekbone surgery, as well as soft tissue cover changes after bone displacement. Material and methods. The study included 16 patients. Of these, 9 ones underwent cheekbone augmentation surgery, 7 patients — cheekbone reduction surgery. Seven points were selected on axial scans passing through the zygomatic arch. In these points, we analyzed zygomatic bone and soft tissue displacement early after surgery and in long-term period. Results. Zygomatic bone and soft tissue movement patterns following augmentation cheekbone surgery are described. We also estimated zygomatic bone and arch movement patterns following reduction cheekbone surgery. Conclusions. Augmentation cheekbone surgery can significantly (78.7% of intraoperative displacement) increase zygomatic volume in anterolateral region and practically does not affect the volume in posterior region. Reduction cheekbone surgery with cutting of zygomatic arch evenly reduces the volume of the entire zygomatic area. Reduction cheekbone surgery without cutting the zygomatic arch reduces zygomatic volume only in anterolateral region.
