TRATAMIENTO ACTUALIZADO DEL DENGUE EN EL PARAGUAY
The updated treatment of dengue fever, according to the guidelines of the Ministry of Health, focuses on adequate fluid replacement, based on the clinical staging of the disease (groups A, B, C and D). Treatment prioritizes the early identification of warning signs, such as severe abdominal pain, persistent vomiting and mucosal bleeding. Patients in group A, without warning signs, can be treated at home with oral hydration and daily monitoring. Groups B and C, with warning signs and special clinical conditions, require hospitalization and strict monitoring, including intravenous fluid replacement. Hydration management takes into account the clinical condition and comorbidities, such as cardiovascular disease and diabetes. In group D, patients with shock require intensive care and hemodynamic support for stabilization. The protocol highlights the importance of monitoring parameters such as hematocrit and platelets, in addition to performing complementary exams according to clinical evolution, including laboratory tests to confirm the diagnosis. Special attention is given to vulnerable populations, such as the elderly and pregnant women, who require constant monitoring due to the high risk of unfavorable outcomes. For pregnant women, clinical management is individualized, prioritizing the safety of the mother and fetus. The guidelines emphasize the need for immediate notification to surveillance agencies in confirmed or suspected cases, aiming to prevent outbreaks and protect public health (1).
