IBC Oxford University Oral Abstract 39 - Use of PSDV-SG in robotic sleeve gastrectomy is safe and effective

Abstract Introduction Despite the reliability and safety of the current generation of surgical staplers, staple line complications can still occur and may result in significant morbidity and mortality. While the incidence of haemorrhage and leak is relatively low (1–6% for bleeding, 1–6% for leakage) the results can have serious implications for the patient and high costs for the facility. The Peri-Strips Dry Staple Line Reinforcement with Veritas Collagen Matrix is derived from bovine pericardium to produce a non-crosslinked collagen matrix that has been shown to reduce the risk of staple line bleeding and leaks. In 2019, the FDA approved Peri-Strips Dry Staple Line Reinforcement with Veritas Collagen Matrix with Secure Grip Technology (PSDV-SG). This new product may allow for greater ease of use and reduced preparation time. The aim of this study is to demonstrate that buttressing with PSDV-SG will reduce staple line complications in robotic sleeve gastrectomy and is safe and effective. Methods We conducted a retrospective study of 60 patients who underwent robotic-assisted sleeve gastrectomy with buttressing using PSDV-SG between May 2020 and May 2022 at a single institution. We evaluated the patients' charts for post-operative leak rates, presence of staple line bleeding, blood transfusions, re-admission rates, and mortality. Results A total of 60 patients were identified, with a 100% follow-up rate within 1 month and 91% follow-up rate over a period of 3–6 months. The majority of the patients were female and from underrepresented minority populations. There were no cases of postoperative leaks or staple line bleeding, and no patients required blood transfusions. One patient was readmitted due to nausea and vomiting, and there were no instances of reoperation or mortality in the study. Conclusion Safety and precision of stapling devices in bariatric surgery is crucial to avoid staple line complications which are associated with increased morbidity, mortality and costs. PSDV-SG is a product that is made from the same biologic material as PSDV but is applied to the surgical stapler via a pressure-sensitive adhesive strip which is time-saving and efficient. Our study demonstrated that the application of PSDV-SG is safe and effective in patients undergoing robotic sleeve gastrectomy.
