Extensive skin necrosis secondary to idiopathic thrombotic vasculopathy associated with ulcerative colitis

Cutaneous and mucosal manifestations associated with inflammatory bowel disease are frequent. We report three patients with ulcerative colitis who presented with extensive retiform purpura and/or cutaneous necrosis secondary to idiopathic thrombotic vasculopathy of the dermis. A literature review revealed six similar cases of thrombotic vasculopathy in the context of ulcerative colitis. Diagnosis requires a skin biopsy and the exclusion of alternative diagnoses. Empirical treatment includes corticosteroids, anticoagulation and management of the ulcerative colitis, and has a favourable prognosis and low risk of recurrence. We believe that clinicians should be aware of this entity, which can sometimes serve as a revealing feature of ulcerative colitis.
