Systematic-Quantitative Analysis of Dapsone Treatment in Patients With Refractory Herpes Gestationis: Impact on Pregnancy Progression and Neonatal Outcomes

Objective: This study aims to systematically and quantitatively analyze the impact of dapsone treatment in patients with refractory herpes gestationis (HG), focusing on its effectiveness in improving pregnancy outcomes and neonatal health. Theoretical Framework: Herpes gestationis is a rare autoimmune blistering disorder that poses significant challenges in pregnancy management. While systemic corticosteroids remain the standard treatment, refractory cases necessitate alternative therapeutic approaches. Dapsone, known for its anti-inflammatory and immunomodulatory properties, has shown potential in achieving disease control in corticosteroid-resistant HG cases. Method: A systematic review was conducted by searching PubMed, Scopus, and Web of Science databases for studies published between 2016 and 2023. The inclusion criteria comprised studies that provided data on maternal and neonatal outcomes following dapsone treatment for refractory HG. Exclusion criteria eliminated studies with insufficient outcome reporting, unrelated conditions, and small sample sizes. Results and Discussion: The review identified promising outcomes associated with dapsone use in refractory HG, demonstrating symptom relief and improved maternal health. However, potential adverse effects, such as hemolysis and methemoglobinemia, necessitate careful monitoring. The findings emphasize the importance of individualized treatment strategies and multidisciplinary care to optimize outcomes. Research Implications: This study underscores the need for standardized protocols and large-scale clinical trials to establish the safety and efficacy of dapsone in pregnant women with HG. Further research should explore long-term neonatal health outcomes and refine treatment guidelines. Originality/Value: This review contributes to the limited literature on dapsone use in pregnancy, offering insights into its potential role as a second-line treatment in refractory HG cases and guiding future clinical practices.
