Interest of Distal Urethroplasty with Posterior Urethrostomy in the Management of Multioperated Proximal Hypospadias

The study aims to evaluate the effectiveness of distal urethroplasty with a perineal fistula in the treatment of multi-operated posterior hypospadias. Materials and Methods: This was a prospective monocentric study involving a heterogeneous series of 16 children with proximal hypospadias who had undergone multiple surgical interventions. All children were operated on by the same surgeon using our approach, which involved performing distal urethroplasty with the creation of a perineal urethral fistula, which was closed six months later. The average age of the children was 7.4 years (88.5 months). Functional outcomes were assessed, and a questionnaire was administered to measure satisfaction among patients, parents, and the surgeon regarding functional and aesthetic aspects. Results: We observed two partial failures of distal urethroplasty after the first surgical phase. A urethrocutaneous fistula was noted in one child after the closure of the urethrostomy. Conclusion: Distal urethroplasty, combined with posterior urethrostomy, represents an excellent alternative for managing severe hypospadias that have undergone multiple surgical interventions. This approach enhances the success rates of urethroplasty by allowing urine diversion during the initial surgical phase. Additionally, closure of the posterior fistula typically proceeds without complications, owing to the favorable quality of the urethral plate in this region and the availability of intermediate layers that ensure adequate coverage during the urethrostomy procedure.
