Current status of health management for elderly chronic disease patients in China and optimization of the pathway based on the four-dimensional linkage of “hospital-community-family-individual”

This study investigated the current application status of health management for chronic diseases among the elderly. Based on the four-dimensional linkage of "hospital - community - family - individual", it put forward targeted path optimization suggestions in terms of government legislative guarantee, information sharing, primary medical care and individual self-management, in order to actively cope with the new challenges of health management for chronic diseases in the elderly and enhance the quality of life of the elderly.
