Surgical outcomes of «inferior» recurrent retinal detachment complicated by advanced stages of anterior proliferative vitreoretinopathy using intraoperative and short-term postoperative perfluorocarbon liquid endotamponade

Purpose. To evaluate results of surgical treatment of «inferior» recurrent rhegmatogenous retinal detachment (RRD), complicated by advanced stage of proliferative vitreoretinopathy (PVR) in «light» silicone oil-filled eyes (SO), depending on the option of using perfluorocarbon liquid (PFCL): vitreoretinal surgery in combination with retinotomy (RT) and intraoperative PFCL endotamponade (vitreoretinal surgery+RT+intraoperative PFCL) and vitreoretinal surgery in combination with RT and short-term postoperative PFCL endotamponade (vitreoretinal surgery+RT+postoperative PFCL). Material and methods. Results of surgical treatment of 66 patients with «inferior» recurrent RRD in silicone oil-filled eyes by advanced stage of PVR were analyzed. All patients were divided into 2 groups depending on the tactics of using PFCL: vitreoretinal surgery+RT+intraoperative PFCL+SO (36 eyes) and vitreoretinal surgery+RT+postoperative PFCL+SO (30 eyes). Results. After surgical treatment, anatomical success was similar in both groups with no statistically significant difference (p=0.5). When comparing bestcorrected visual acuity (BCVA) between the two groups 3 months after surgery, visual acuity was significantly higher in group 2 compared to group 1 — 0.05 versus 0.03 (p=0.016). During the observation period of 6 months after the analyzed operations and after removal of the SO, visual acuity was also significantly higher in group 2 compared to group 1 (p=0.01). Conclusion. In case of «inferior» recurret RRD complicated by advanced stage of PVR in silicone oil-filled eyes, both vitreoretinal surgery in combination with RT and intraoperative PFCL endotamponade and vitreoretinal surgery in combination with RT and short-term postoperative PFCL endotamponade allow achieving the same anatomical results. Vitreoretinal surgery combined with RT and short-term postoperative PFCL endotamponade may be the best choice to achieve a better functional outcome. Key words: recurrence of rhegmatogenous retinal detachment, retinotomy, vitreoretinal surgery, silicone oil, perfluorocarbon liquid, advanced stage of proliferative vitreoretinopathy.
