A Hybrid Operational Risk Assessment Method Combining System Failure and Human Errors in Civil Aircraft
Safety is always the first concern for public when it comes to airplanes. Thanks to the efforts made by all related stakeholders, taking an airplane now is significantly much safer than it was first invented. However, in this ever-changing industry, practitioners are still striving a lot to manage operational risks in order to maintain or enhance the safety level. In this paper, a hybrid operational risk assessment method is introduced that combines information from safety design, operational data used to calculate the dynamic operational failure probability, and non-normal procedures used to perform human error evaluation, which is derived from cognitive reliability and error analysis method (CREAM), to provide an insightful and overall risk picture for safety practitioners within the civil aviation industry.
