Navigating the Deep Venous Obstruction: To Stent or not Across the Hip Joint?
Results: A total of 65,090 superficial venous procedures encompassing endovenous thermal ablations, stripping/high ligation, and microphlebectomy were included.Baseline demographics, clinical characteristics, and CEAP classifications are shown in Table I.NHW patients underwent interventions for less severe baseline SVI based on CEAP class and had more superficial venous interventions (P < .001)and repeat thermal ablations (P < .001)than other groups.NHW patients were also more likely to have concomitant microphlebectomy than NHB patients (P < .001).NHB had more severe baseline SVI based on CEAP class than other groups.Perioperative quality-of-life data are shown in Table II.NHB had the highest VCSS score preoperatively (8.17 6 4.02) with the largest improvement at #3 (4.40 6 5.23) and >3 months (7.00 6 5.00) following intervention.Hispanic/Latinos had the highest preoperative HASTI score (10.34 6 5.40) and the largest score reduction at #3 months (6.62 6 6.51).There were no significant differences in systemic, hematologic, or dermatologic complications between groups (P NS).Conclusions: Minority racial/ethnic groups have more clinically severe venous disease at presentation compared with NHW patients, but once treated, they exhibit significant symptomatic improvement.Minority groups undergo fewer adjunctive/repeat interventions than NHW while achieving similar clinical success. There may be disparities in care seeking, access to care, patient selection, and referral pathways that underlie these treatment patterns and outcomes, requiring further study.
