SUBSTANTIATION OF THE EFFECTIVENESS OF ENTEROSORPTION IN PATIENTS WITH NEUROISCHEMIC FORMS OF DIABETIC FOOT SYNDROME
Abstract. One of the causes of diabetes mellitus (DM) is an increase in life expectancy, an aging society, the presence of obesity, and others. 5-7 % of the adult population suffers from diabetes. According to literature sources, every 10-15 years the number of patients with diabetes mellitus increases by approximately 2 times. With diabetes, there is a high probability of a serious complication - diabetic foot syndrome (DFS), which develops in 6–11 % of patients and 40–70 % of them require surgical treatment. In the presence of SDS, the intoxication of the body also increases due to the purulent-inflammatory or necrotic process, which proves the need for detoxification of the body in patients with SDS. Object and research methods. In 2021, we treated 121 patients with various stages of SDS on the basis of KNP KMKL No. 6 in Kyiv. The study included 21 patients with SDS with trophic ulcers, phlegmons of the foot and wounds after amputation of one or more toes. Oral sorbent «Enterosgel» was used for the purpose of detoxification. The results. As a result of the conducted studies, the effectiveness of enterosorption with enterosgel in diabetic foot syndrome of the neuroischemic form was shown. Conclusions. As a result of the conducted research, the effectiveness of xerogel absorption of polymethylsiloxane was shown; so the functional activity of the effector cells of the natural resistance system of these cells in the induced test on the 5th day increased by 1.71 times in relation to the initial values (р<0.5); an increase in the effective concentration of endogenous albumin by 1.56 times was established (p<0.01); The toxin-binding capacity of serum albumin increased by 1.25 times (p<0.05); the content of MSM decreased by 1.64 times (p<0.05); the indicator of body intoxication decreased by 1.21 times compared to the initial data. In this way, the effectiveness of enterosorption with polymethylsiloxane xerogel in diabetic foot syndrome of the neuroischemic form is clearly shown, which contributes to improving the results of treatment of patients with SDS.
