Secure and Interoperable IoMT-Based Smart Homes

The integration of Internet of Medical Things (IoMT) devices is critical for enabling intelligent healthcare services in smart homes. However, centralized access control methods, such as OAuth 2.0, are vulnerable to token leakage, session fixation, and unauthorized access. This study proposes a decentralized architecture leveraging trigger–action platforms (TAPs), blockchain, and decentralized identifiers to enhance security and interoperability. TAPs validate credentials using blockchain-anchored verifiable credentials, blocking unauthorized access to health data and ensuring only authorized caregivers can retrieve sensitive information. They also enable seamless interoperability, such as connecting a health monitoring device with a home automation system to adjust room temperature based on patient vitals. By addressing single points of failure inherent in centralized systems, the proposed system ensures scalable identity management and tamper-resistant access control. A proof-of-concept implementation in a healthcare smart home validates its effectiveness, demonstrating enhanced security, scalability, and operational reliability in IoMT ecosystems.
