Comparison Of Nebulized Lignocaine With Dexmedetomidine VersusNebulized Lignocaine With Clonidine In Obtunding The Laryngoscopic Responses -Prospective, Randomized, Double-Blind Study.

Background:The most prominent autonomic responses elicited by endotracheal intubation may give rise to potentially hazardous cardiovascular complications, particularly in patients with pre-existing heart diseases or conditions.Autonomic responses can be mitigated by administering local anaesthetics, mostly lignocaine, by inhibiting afferent nerve stimulation and decreasing the effects of such reactions.Adjuvant agents, the alpha-2 adrenergic agonists dexmedetomidine and clonidine, have only lately been recognized to enhance the overall effects of lignocaine and even further decrease the physiological stress response that may arise from intubation. Objective: This study was aimed to compare the efficacy of nebulized lignocaine 4% with dexmedetomidine 100g versus nebulized lignocaine 4% with clonidine 150 in blunting hemodynamic response during laryngoscopy and endotracheal intubation on heart rate and blood pressure during and after the procedure. Methods: This was a randomized, controlled, double-blind study of 100 adult patients who underwent elective surgery under general anaesthesia with endotracheal intubation. The patients were randomly allocated to two groups. In Group A, patients received 4% lignocaine 4ml with dexmedetomidine 1ml (100g) (Control Group) and Group B (Study Group) received 4% lignocaine 4 ml with clonidine 1ml (150g).The two drug combinations were given by nebulization 15 minutes before induction. All the patient's hemodynamic parameters, like heart rate and mean arterial pressure, were monitored at baseline after nebulization, during laryngoscopy, and at intervals after intubation.Post Operative Sore Throat (POST) and other adverse events were also assessed.
