Stress hyperglycemia in the acute phase of hemorrhagic stroke in Burkina Faso: prevalence and impact on mortality
Background: Stress hyperglycemia is common and associated with poor prognosis in hemorrhagic stroke. The aim of our study was to assess the prevalence of stress hyperglycemia in the acute phase of stroke and its impact on in-hospital mortality. Methods: This was a retrospective study in the medical emergency and neurology departments of the Yalgado Ouedraogo University Hospital from 1 January 2021 to 31 December 2022. Non-diabetic adult AVCH patients with hyperglycemia >=7mmo/l on admission and normal glycated hemoglobin were included. Bivariate and multivariate analysis were used to assess the impact of stress hyperglycemia on mortality. Results: The mean age of the patients was 55.72(SD:13.3)y and the sex ratio was 1.58. Arterial hypertension was the main etiology in 89% of cases. The mortality rate was 23.1%. The prevalence of stress hyperglycemia in the acute phase of stroke was 52.3%. Independent factors associated with in-hospital mortality were admission hyperglycemia >=7mmo/l (p= 0.0017) and NIHSS score >=17 (p= 0.0136). Conclusion: Mortality in stroke remains high. Stress hyperglycemia is a poor prognostic factor in stroke. Early diagnosis and management of stress hyperglycemia and other severity factors will reduce mortality
