Relationship between grading of invasive ductal breast carcinoma at RSPAL Dr. Ramelan Surabaya and tumor infiltrating lymphomocytes (TILs) period January 2020–December 2022

Worldwide, including in Indonesia, breast carcinoma is the most frequent form of cancer among women. Out of 396,914 cancer patients in Indonesia, 68,858 (16.6%) were diagnosed with breast cancer. An essential component of the immune response to cancer, Tumor Infiltrating Lymphocytes (TILs) are receiving more and more attention in studies on breast carcinoma. Groups of immune cells, including T lymphocytes, are discovered in tumor tissue and are referred to as TILs. Prognostic information can be gleaned from TIL existence, composition, and activity, according to substantial evidence. Grading, shows the degree of tumor aggressiveness and is crucial for treatment planning, is an essential part of breast cancer. We still do not completely understand the significance of TILs in deciding grading of invasive ductal carcinoma, one of the most common kinds of breast carcinoma. We set out to document the correlation between TILs and invasive ductal breast cancer grading from 2020 to 2022. The study used a retrospective cross-sectional analysis as its methodology. RSPAL Dr. Ramelan Surabaya surveyed patients diagnosed with invasive ductal carcinoma utilizing a total sampling technique. The patient preparations for invasive ductal carcinoma were cut into paraffin blocks, stained, and then separated into three groups. Patients with grade 1 invasive ductal carcinoma were prepared in group 1, those with grade 2 invasive ductal carcinoma in group 2, and patients with grade 3 invasive ductal carcinoma in group 3. Subsequently, TILs were counted under a microscope for each group. There were 28% TILs in the mild group, 44% in the moderate group, and 28% in the high group of patients with grade 1 invasive ductal breast cancer. Patients with grade 2 invasive ductal breast cancer were categorized as having a low number of TILs 16%, a moderate number 40% or a high number 44%. Subjects with grade 3 invasive ductal breast cancer were categorized as having a low number of TILs 44%, a moderate number 24%, or a high number 32%. The study found a highly significant (p = 0.001) correlation between the TILs variable and the grade of invasive ductal breast cancer. This correlation was shown to be unidirectional.
