Effect of Diabetic Nephropathy on Cardiovascular Autonomic Functions: Systematic Review Article

Diabetic nephropathy (DN), a common microvascular complication of diabetes, is a leading cause of chronic kidney disease (CKD) and end-stage renal disease (ESRD), associated with significant global morbidity and mortality.Beyond its renal implications, DN profoundly impacts cardiovascular health by disrupting autonomic balance. This disruption is exemplified in cardiovascular autonomic neuropathy (CAN), a severe diabetes complication that impairs heart rate variability, blood pressure regulation, and vascular tone. The coexistence of DN and CAN amplifies cardiovascular risk through a vicious cycle of systemic inflammation, oxidative stress, and neurovascular damage, making cardiovascular disease (CVD) a prevalent complication in diabetes. This systematic review synthesizes evidence from 32 studies to explore cardiovascular autonomic dysfunction in DN, employing a PRISMA-based methodology. It highlights mechanisms like uremic toxin accumulation, endothelial dysfunction, and heightened sympathetic activity, which contribute to autonomic dysregulation.Clinical manifestations include reduced heart rate variability, impaired baroreflex sensitivity, orthostatic hypotension, and resting tachycardia, which increase cardiovascular morbidity and mortality.Early diagnosis and targeted interventions for DN and CAN are emphasized.Comprehensive management strategies discussed include lifestyle modifications, glycemic control, pharmacotherapy, and advanced technologies such as renal denervation and neuromodulation. The review also underscores research gaps, such as the need for biomarkers for early detection and long-term evaluation of new treatments.Addressing these gaps could enhance patient outcomes and reduce the global burden of diabetes-related complications, particularly in individuals at high cardiovascular risk.
