Evaluate the Impact of Psychosomatic Factors in Oral Lichen Planus and Recurrent Apthous Stomatitis. A Cross sectional Study

The mouth is directly or symbolically related to major human instincts and is charged with a high psychological potential, few important of them being lichen planus and apthous ulcer.Aim & Objective:To evaluate level of stress in patients with apthous ulcer and lichen planus by health questionnaires HAD scale. To compare serum cortisol level with the occurrence of apthous ulcer and lichen planus. Correlation between serum cortisol level and HAD scale which assess the stress and anxiety.Material & Methods: Total of 50 patients were selected which comprised of 20 clinically and Histopathologically diagnosed oral lichen planus patients as study group. 20 clinically diagnosed Apthous ulcer patients as study group. 10 healthy individuals with age- and sex-matched were taken as control group. A questionnaire was used to evaluate stress, anxiety and depression in all patients. Hospital Anxiety and Depression Scale (HADS) with rating 0-3. Serum cortisol level estimation in all patients in morning timeResults:The analysis of our results points out an association between psychogenic alterations due to stress, anxiety and depression lead to changes of the oral mucosa, particularly in conditions like RAS and OLP. The comparison of results of the study and control groups showed that subjects 80% of study group which comprised of apthous ulcer and oral lichen planus subjects were having stress, anxiety and depression as compared to 30% subjects of control group  with a P value of 0.001 which is highly significant.
