Radionuclide methods in analysis of myocardial perfusion and metabolic activity

Objective. To compare the effectiveness of combined diagnostic algorithm for coronary artery disease involving myocardial PET/CT with 18F-FDG and PET/CT with 82Rb-chloride, as well as algorithm including myocardial PET/CT with 18F-FDG and SPECT/CT with 99mTc-chloride. Material and methods. The study involved 24 patients aged 34—60 years with coronary artery disease. All patients underwent perfusion SPECT/CT with 99mTc-Technetrile at rest and stress test (bicycle ergometer), as well as metabolic PET/CT with 18F-FDG. A protocol for perfusion PET/CT with 82Rb-chloride was developed. Results. SPECT revealed signs of focal cicatricial changes in 4 patients and signs of myocardial perfusion defect in 13 patients. Contractility impairment zones were observed in 14 patients. When analyzing the PET/CT data, areas of reduced metabolism partially or completely corresponding to perfusion deficiency were detected in 16 patients. Conclusion. These data completely coincided with angiography results. We plan the final stage of the study after receiving the 82Sr/82Rb generator.
