Objectives: Preeclampsia (PE) have variable atypical clinical presentations, unpredictable course with adverse pregnancy outcomes. It's hallmark features are high blood pressure and endothelial dysfunction, leading to widespread end-organ injury, new-onset hypertension and newonset of altered angiogenic factors (sFlt-1/PlGF ratio or PlGF alone). It is one of the important cause of MMR across the world & significantly contributes to maternal, perinatal morbidity and mortality. An effective predictive test for preeclampsia would facilitate prevention, early diagnosis, targeted surveillance and timely delivery. Methods: Till now no single test can comprehensively predict the risk of PE.Results: Maternal characteristics are identified as high risk factors for PE by NICE 2013 & USPSTF, MFM and Gestosis society. The USPSTF maternal characteristics recently been adopted by ACOG 2018 and they commented, these guidelines are applicable, even in low-resource settings.Specific maternal characteristics scoring system if combined with Uterine Doppler study increase the predictive value.Alteration of Biomarkers in blood (PAPP, PLGF, Tyrosin kinase-1, and sFLt/ PLGF) are associated with an increased risk of preeclampsia. So it's addition to that increase the sensitivity and specificity. Conclusions: Regarding Prevention, there is no magic bullet but general recommendations are-Losing weight, quitting smoking having regular exercise, routine taking aspirin daily from 12 wk gestation onward, consuming high dose of Calcium, in risk group and regular checkup of BP at antenatal checkup may prevent the progression of diseases.Similarly treatment of hypertension, other comorbidities and timed delivery may prevent the serious consequences of PE.
