THE MODIFIED KONNO PROCEDURE IN PEDIATRIC OBSTRUCTIVE HYPERTROPHIC CARDIOMYOPATHY
Background and Aim: Transaortic septal myectomy is the preferred surgical treatment for hypertrophic cardiomyopathy (HOCM) in children. This study assesses outcomes of modified Konno in children with severe HOCM. Methods: This study retrospectively analyzed outcomes in 10 children who underwent modified Konno procedure in our institute between May 2021 and July 2024. The primary outcomes measured included survival rates, early morbidity, and mid-term functional as well as echocardiographic status. Results: The median age at the time of the operation was 4.64 years (IQR: 2.5-14.2), and the median weight was 21 kg (IQR: 14-47). One patient had undergone a prior procedure. Mean septal thickness was more than 25 mm. LVOT mean gradient 55 mmHg. Post procedure LVOT mean Gradient was 5 mmHg. There was one early death. One patient needed a permanent pacemaker due to complete heart block. Conclusions: The modified Konno procedure effectively relieves obstruction and ensures good survival in children with severe obstructive hypertrophic cardiomyopathy.
