12 Innovative care delivery model: increasing equity through novel post acute clinic

Description
Healthcare disparities persist, particularly in marginalized communities with inadequate follow-up care. Our clinics aim to shorten hospital stays and enhance post-discharge support, with funding from our hospital that eliminates payer barriers. We evaluated a new multidisciplinary post-acute care model for its effectiveness in reducing hospital stays and improving patient outcomes while promoting equitable healthcare. From March 1 to April 28, 2024, we saved 212 hospital bed days, providing prompt intravenous therapies and follow-up care within 7 days. These clinics became vital for communities lacking follow-up, addressing care gaps and promoting accessible, high-quality services.
