35 Hardwiring psychological safety as the foundation for a successful quality and safety program

Description
Radiology departments have unique challenges with regards to patient safety due to the high volume of patient visits and advanced image guided procedures referred by outside providers, the critical safety challenges of Magnetic Resonance Imaging (MRI), and the uncoupling of physicians from the site of service. Improving psychological safety can impact employee satisfaction. The purpose of this session is to describe the multifaceted approach to overcome these barriers in our department which have been successfully built upon a foundation of psychological safety promoting vigilant safety event reporting and review using the principles of a just culture. We will discuss the approach in our department highlighting specific improvement initiatives including MRI safety, a resident centered mock Root Cause Analysis (RCA) event, a procedural time-out initiative, and an overview of a customized safety event reporting system. Results for each initiative will be discussed and reviewed. Radiology presents a unique set of patient safety challenges with potential for harm. A successful safety program built on a foundation of psychological safety can mitigate these risks and impact employee burnout.
