The coffee equation: balancing benefits and risks

Caffeine which is 1,3,7-trimethyl xanthine is consumed all over the world and it must be the one of the oldest beverages found[1]. The psychostimulant affects our body through its various components such as caffeic acid, quinic acid, chlorogenic acid, coumarins, flavonoids, and anthocyanins (Fig. 1)[1]. Caffeine offers cognitive enhancement, physical performance, neuroprotection, increased cardiovascular function, including addiction, stomach upset, acid reflux, anxiety, and jitteriness[1]. It is essential to balance these extremes and experience the benefits of it. Enjoyment, optimized performance, and increased wakefulness are the main motivators of coffee consumption[1].Figure 1.: Various components in coffee. Popular associations have been made to arrhythmia and coffee consumption, caffeine inhibits phosphodiesterase leading to increased cAMP levels stimulating protein kinase A and release of more calcium from sarcoplasmic reticulum potentially a pro-arrhythmogenic function[2]. Caffeine consumption, equivalent to one cup of espresso, causes a significant increase in plasma caffeine levels within the first 60 min[3]. This increase leads to peripheral vasoconstriction, resulting in a modest rise in blood pressure (approximately 7 mmHg over 120 min)[3]. Simultaneously, heart rate decreases slightly due to stable cardiovagal baroreflex activity[3]. Notably, the increase in blood pressure is not mediated by increased cardiac output or sympathetic vasoconstrictor activity in skeletal muscle[3]. Instead, vasoconstriction occurs secondary to direct blockade of adenosine receptors in peripheral vascular beds[3]. The long-term effects of coffee have been beneficial, chlorogenic acid, quercetin, and other compounds may enhance endothelial function, promoting vasodilation[4]. Chlorogenic acid, this polyphenol is abundant in coffee, inhibits angiotensin-converting enzyme (ACE), reducing vasoconstriction also enhances nitric oxide production, promoting vasodilation[4]. Quercetina This flavonoid may inhibits xanthine oxidase, reducing oxidative stress, activates potassium channels, promoting vasodilation in addition it also inhibits inflammatory pathways[4]. Coffee on the contrary also contains noncaffeine compounds, such as cafestol, a diterpene molecule made without paper filtering linked to cardiovascular risk[5]. It boosts low-density lipoprotein cholesterol levels and is associated with coronary artery disease.[5] When consumed, caffeine blocks adenosine receptors on the central nervous system which increases neurotransmitters like dopamine, norepinephrine and serotonin, this leads to activation of sympathetic system, linking to anxiety[6]. At the same time increased dopamine and norepinephrine enhances cognitive function. The chlorogenic acid and polyphenols acts as a antioxidant and anti-inflammatory agent and helps protection against Alzheimer’s by reducing amyloid-β deposition[7]. On the same lines adenosine receptor antagonism increased sympathetic tone, vasopressor effects may lead to increased intraocular pressure[8]. The adenosine receptor antagonism also explores increases insulin resistance leading to decreased glucose uptake and glycogen synthase activity in skeletal muscle[9]. Quinide a metabolite of chlorogenic acid increases insulin sensitivity increasing glucose uptake in skeletal muscle[9]. Coffee consumption lowers the risk of gall stone disease by increasing bole flow and cholesterol saturation[10]. Caffeine can regulate gut motility by increasing release of gastrin and cholesystokinin[10]. The chlorogenic acid have anti-oxidant and anti-inflammatory effects in addition to it can modulate gut microbiome increasing the beneficial bacteria reducing pathogenic bacteria[10]. Polyphenols in coffee reduces oxidative stress and inflammation slowing chronic kidney disease, it has also diuretic effect leading to decreasing in chances of kidney stones[11]. Coffee, a favorite beverage, has faced a lot of debate regarding its effects. We thus recommend individualizing the coffee dose to safe limits and not overdoing the usage. Nothing is more important than a sound body, and coffee might be a great beverage for leisure, but it is not something to exploit for enhancing work capacity. We strongly recommend respecting the limits of one’s body and seeking medical advice for individualized guidance in case someone faces symptoms like insomnia, palpitations, or anxiety.
