First Trimester Use of Buprenorphine or Methadone and the Risk of Congenital Malformations

( JAMA Intern Med . 2024;184(3):242–251. doi: 10.1001/jamainternmed.2023.6986) For pregnant patients with opioid use disorder (OUD), buprenorphine and methadone are prescribed to lower the risk of overdose and prevent withdrawal symptoms. Studies have shown a lower risk of preterm birth, low birth weight, and neonatal abstinence syndrome using buprenorphine compared with methadone. There have not been large studies to understand the risk of congenital malformations. This retrospective cohort study looked at data from a national Medicaid database on first-trimester pregnancies from 2000 to 2018. The goal was to assess the comparative risk of congenital malformations among infants exposed to buprenorphine compared with methadone.
